A DNR decision might be different for an otherwise healthy year old, than for an year old with multiple health issues. POLST and similar programs involve a physician-initiated discussion and shared decision-making process with people with advanced or end-stage illness. In a medical crisis, emergency medical technicians and other health care practitioners should first follow POLST.
If immediate action is not essential, POLST should be reviewed with the person whenever the person's condition changes significantly, the person's venue of care changes, or the person chooses to change his or her wishes. For people lacking decision-making capacity, their authorized surrogates can act on their behalf. Some states require a specific colored paper copy of your order to be posted on a refrigerator or other visible location.
If is called to attend to a person who has collapsed, emergency medical services EMS will check for an out-of-hospital DNR order with original signatures or whether the person is wearing an out-of-hospital DNR bracelet or necklace. This is the law in all communities. An attending physician will not issue an out-of-hospital DNR order if the person requesting the order is not eligible for it.
Most physicians will only sign such a document for a person who is terminally ill or extremely elderly and frail, and mentally capable of making such a request. EMS providers are not authorized to comply with a bracelet or necklace that has not been ordered by a physician. Regulations vary by state, so check with your doctor about availability of a DNR bracelet. Many people fear CPR or becoming debilitated if they are resuscitated. If you are a healthy individual there are many scenarios when resuscitation would return you to a normal, productive life.
